What Information do we collect and when?
When you are using Callondoc.com Products and Services, we may need to collect data to provide you the Service as described in the Callondoc’s Privacy Policy. Our intake form asks users to enter sensitive information (such as personal health information or credit card number). Telemedicine visit
A telemedicine visit may require personal permissions to your advice, such as getting SMS information and phone status, while we only can access them under your allowance. The main purpose of this is to display data on the smartwatch and hang up the phone service through the watch. There is no other malicious intent and no violation of our user data policy on personal and sensitive information. We strictly follow the associated rule of personal information protection and privacy rights.

Callondoc.com may process your personal data and health history for the following purposes. One or more purposes may apply simultaneously.
For communication: we may use your email to communicate with you, for example to inform your medication has been e-prescribed.

Our Responsibilities
CallonDoc.com manages patient's Information in compliance with HIPAA Rules and U.S. Department of Health & Human Services (HSS).
Disclosure of Information: CallonDoc Inc will not use or disclose patient's Information in any way that is prohibited by law.
Securing of Information: CallonDoc Inc will secure patient's Information to prevent unauthorized use or disclosure, including implementing requirements of the HIPAA Rules.
Breach of Information: CallonDoc Inc will report to You any breaches of Your Information.
Account of Information request: CallonDoc Inc will communicate when a patient requests an account of their Information and assist in responding, completing or denying the request.
Subcontractors Compliance: CallonDoc Inc will ensure that any subcontractors it may engage with that have access to Your Information will agree to the same restrictions and conditions that apply to CallonDoc Inc with respect to You or Your Information.
Your HIPAA Rules Obligations: CallonDoc Inc will carry out Your obligation under the HIPAA Rules that require CallonDoc Inc to comply with the requirements applicable to the obligation.
Your Health & Human Services (HHS) Compliance: CallonDoc Inc will make available to the Department of Health and Human Services (HHS) Our internal practices, books, and records relating to the use and disclosure of Protected Health Information (PHI) received from, created, or received by CallonDoc Inc on behalf of You.
Your Health Information Rights
The health and billing records we maintain are the physical property of Callondoc.com. You have the right to:
Request a restriction on certain uses and disclosures of your health information by delivering the request to our office--we are not required to grant the request, but we will comply with any request we do decide to grant;
Obtain a paper copy of the current Notice of Privacy Practices for Protected Health Information (“Notice”) by making a request at our office;
Request that you be allowed to inspect and copy your health record and billing record--you may exercise this right by delivering the request to our office;
Appeal any denial of access to your protected health information, except in certain circumstances;
Request that your health care record be amended to correct incomplete or incorrect information by delivering a request to our office. If your request is denied, you will be informed of the reason for the denial and will have an opportunity to submit a statement of disagreement to be maintained with your records;
Request that communication of your health information be made by alternative means or at an alternative location by delivering the request in writing to our office;
Obtain an accounting of certain disclosures of your health information that we are required to maintain by delivering a request to our office. An accounting will not include uses and disclosures of information for treatment, payment, or operations; disclosures or uses made

To Request Account Deletion
By utilizing the services of callondoc.com, it is a requirement and necessity for our providers to have access to the information collected to be able to safely review and provide medical treatment in accordance to best practices and medical guidelines.
Information we are collecting includes but is not limited to: Name, Email, Phone Number, Gender, Race as well as device information such as Device ID, Device Name, Device OS Version.
Callondoc.com collects information about the device used to access our services to provide the most up-to-date version of our products and services on your specific device for ease of use and to follow device updates from the manufacturer.
To delete your account, simply access the Profile section on Call-On-Doc app and delete it or you can connect with us for the account deletion. You can also recover your account by just signing in with your credentials and two factor authentication or you can connect with us for account activation.
Your Responsibilities
1. Minimum Information to CallonDoc Inc
You are responsible to provide CallonDoc Inc only the minimum Personal Information and Health history necessary to accomplish the Service.
2. Follow HIPAA Rules
You are responsible for using administrative, physical, and technical safeguards at all times to maintain and ensure the confidentiality, privacy, and security of Information transmitted to CallonDoc Inc, in accordance with the standards and requirements of HIPAA Rules.
3. Requests that violate HIPAA Rules
You cannot request CallonDoc Inc to use or disclose Information in any manner that would violate the Code of Federal Regulations 45 CFR Part 164 subpart E.

I hereby consent and authorize Callondoc's medical providers (which may include Medical doctors, Physician Assistants, Nurse Practitioners, and Medical Assistants) to administer and perform medical evaluation and treatment deemed necessary and release Callondoc.com Inc of any legal responsibility incurred by the medical providers.
I understand that electronic communication technology will be used during online consultation, and such a consultation will not be the same as a direct patient-health care provider visit due to the fact that I will not be in the same room as my health care provider.
I understand there are potential risks to this technology, including interruptions, unauthorized access, and technical difficulties.
I understand that my healthcare information may be shared with the Callondoc’s medical providers and pharmacy staff for the purpose of providing appropriate medication and treatment plans. Callondoc’s medical providers will all maintain the confidentiality of the information obtained.
Telemedicine or Telehealth carries the risk of misdiagnosis or delayed diagnosis, which could lead to patients not receiving the proper treatment or even receiving the wrong treatment. I understand these potential risks and am willing to proceed with a consultation with Callondoc.
Due to the potential risk of misdiagnosis or delayed diagnosis, I understand and agree to seek immediate medical care if my symptoms do not improve within a timely manner (typically within 48 hours)
I understand that the treatment, therapy or recommendations provided by Callondoc's medical team is an initial and first-line approach and I agree to follow up with a doctor in-person for a re-evaluation as needed.
I understand that the health care provider or I can discontinue the telemedicine visit if it is felt that the electronic communication technology is not adequate for the situation.
I understand that I have the right to withdraw my consent to the use of telemedicine or telehealth at any time in the course of care. As long as the consent is in force, Callondoc medical team may provide health care services to me via telemedicine without the need to sign another consent form.

I have had the alternatives to a telemedicine or telehealth consultation explained to me, and I'm choosing to participate in a telemedicine or telehealth consultation. I certify that I must be an adult patient or an adult legal guardian of a minor patient to use the Callondoc.com platform.
I understand that services rendered by Callondoc are provided on a non-refundable basis.
I understand that my payment to Callondoc.com, the consultation fee, may not cover the prescribed medication and I still have to pay for the prescribe medication at the pharmacy.
I understand that the information given on the medical intake form must be complete, accurate and up-to-date to the best of my knowledge.
I understand that my failure to provide a complete, accurate and truthful information on the intake form puts me at a harmful risk of misdiagnosis and incomplete treatment.
I understand that Callondoc reserves the right to decline treatment if misleading pieces of information are given by the patient or user.
I understand that telemedicine or telehealth is the use of electronic information and communication technologies by a health care provider to deliver services to an individual when he/she is located at a different site than the provider.
I understand that telemedicine or telehealth visits are reserved for mild to moderate conditions and may not be appropriate for severe or life-threatening illnesses.
I understand that medical evaluation, diagnosis and treatment offered on Callondoc.com are virtual or asynchronous in the absence of a face to face physical examination.
I agree to call 911 or seek emergency care if your symptoms or condition worsen or immediate medical is required after your telemedicine visit.
I agree to continue the recommended routine physical visit with an in-person physician while utilizing telemedicine as secondary means of accessing healthcare.
I certify that I do not have any cognitive impairment and capable of making sound medical decisions.
I understand that I'm engaging in telemedicine (telehealth) consultation and I accept the risk of misdiagnosis due to the absence of in-person evaluation or diagnostic tools.
I certify that I must be an adult patient or an adult legal guardian of a minor patient to use the Callondoc.com platform.
I understand that services rendered by Callondoc are provided on a non-refundable basis.
I understand that my payment to Callondoc.com , the consultation fee, may not cover the prescribed medication and I still have to pay for the prescribe medication at the pharmacy.
I understand that I must provide Callondoc with the most accurate and up to date information about my health, medical history, medications, allergy and diagnostic reports to avoid misdiagnosis.
I understand the health history, medical history, medication list, allergy list, diagnostic reports and any other information I provide to Callondoc.com and its representative will be used to evaluate and provide my assessment and treatment plan.
I understand that, by failure to obtain or provide requested diagnostic lab or images studies may increase the risk of misdiagnosis and treatment failure. I will not hold Callondoc and its representatives responsible for any misdiagnosis and treatment failure caused by failure to furnish or comply with medically necessary diagnostic studies.
I hereby acknowledge that I'll follow-up with a doctor in-person for a face to face to re-evaluation if my symptoms worsen or do not improve timely after this telehealth visit.
I understand that the consultation fee covers this only single visit, and acknowledge that a prescription refill request or re-evaluation visit on a different day may incur additional fees and charges.
I have been informed that telemedicine is a supplement to, not a replacement for, in-person physician visits.
I have been informed that electronic communication via email or other asynchronous electronic medical record transfer may delay the delivery of care via telemedicine. However, it's my responsibility to request a cancellation in order to seek immediate care elsewhere if this delay poses a health risk.
I understand that I can cancel the telemedicine visit prior to the consultation status being marked as completed. A complete status is non-refundable.
